Requests Received Before this Date/Time will be discarded.
We are now receiving field allocations which require a few adjustements to practice field Assingments. Please view the assignment sheets regularly to insure your assignment isn't changed. This aspect which is noted on the schedule is out of our control. Thank you for understanding.

1 U7/8 field = 1/2 U9/10 field.
If highlighed then the field access has not been confirmed by the provider
